Visit Find Healthcare Providers: Compare Care Near You | Medicare  website to find and compare nursing homes that participate in Medicare or Medicaid in your area.  The site includes a quality rating system that assigns each nursing home a rating from 1 to 5 stars.  Medicare may impose penalties on a nursing home when serious health or fire safety violations occur or when a nursing home fails to correct identified deficiencies within the required timeframe.

Some nursing homes are licensed by the state but are not certified by Medicare or Medicaid. State-licensed–only facilities that have received state-imposed fines will be listed on this Web site.

Some nursing homes are combination homes. Combination homes offer two levels of care, including skilled nursing, and adult care home. These homes that have had a state fine imposed will be posted on this  Web site.

Effective January 1, 2012, the Centers for Medicare & Medicaid Services (CMS) implemented a formal process allowing eligible entities to apply for funding aimed at improving resident outcomes in certified nursing homes. Only Civil Money Penalty (CMP) fund applications that align with statutory intent, federal regulations, and CMS policy will be considered. Please complete and submit the application for review.
